Instructions for applying for membership:
- You must FIRST be an active member of the West Virginia Public Health Association to be eligible for active membership in the WVPHA-Nursing Section.
- Download, print and complete an application form. You may download the application by clicking here. The application is in Microsoft Word format.
- Dues are $15 if paid prior to April 1st. If paid after this date, dues are $20. Please make check payable to WVPHA-Nursing Section.
- Mail the application along with check or money order to:
